At Amplify Care, we are committed to maintaining the privacy and security of personal health information. We comply with the Personal Health Information Protection Act (PHIPA) and adhere to a series of privacy principles to ensure your data is handled responsibly and safely.

Be accountable

An organization is responsible for personal information under its control and must designate an individual or individuals accountable for compliance with privacy principles.

Identify the purpose

The purposes for which personal information is collected must be identified by the organization at or before the time of collection.

Obtain consent

Knowledge and consent are required for the collection, use, or disclosure of personal information, unless inappropriate.

Information regarding the organization’s privacy policies and practices must be readily available to individuals.

Give individuals access

Individuals have the right to request access to their personal information, challenge its accuracy, and have it amended as necessary.

Provide recourse

An individual must be able to challenge compliance with the privacy principles to the designated accountable individual(s).
