Osteoporosis Assessment Tool for Accuro QHR

Osteoporosis Canada and Amplify Care have collaborated to develop a tool that will assist doctors in best-practice documentation of Osteoporosis care for patients. The tool consists of a main form that links to additional guides and resources. The tool has been developed in accordance with the most recent Osteoporosis guidelines (2024).
Installation Steps:
All Accuro tools can be downloaded from the Accuro Form Cloud:
- In the Form Editor, open the Tools menu.
- Select the dropdown option Publish/Download.
- Search for “Osteoporosis” to view the complete suite of forms.
- Highlight and Download the following forms for Osteoporosis:
- Osteoporosis Management Tool V1.0
