Sick Note Form

In partnership with the Ontario College of Family Physicians (OCFP), we have developed EMR-integrated Sick Note Forms for TELUS PS Suite, OSCAR Pro, and Accuro QHR. 

The OCFP has been advocating that the provincial government amend the Employment Standards Act to prohibit employers from requiring sick notes for short-term illnesses lasting 5 days or less. To help educate employers on changing their policies, we have included a resource in this package from the OCFP that outlines recommendations for sick notes. 

Installation steps for OSCAR Pro:

1. Fill out the form to download the package 
2. Unzip the .zip file to your Desktop 
3. Import the “sick_notes_oscar.zip” file into OSCAR 
4. The original PDF form (“ocfp-writing-sick-notes.pdf”) is included in the package if needed separately
