Evidence2Practice Ontario (E2P) brings together multi-disciplinary, cross-sector expertise under the joint leadership of the Amplify Care, the Centre for Effective Practice, and North York General Hospital (hereafter “program co-leads”). Funding and strategic guidance for E2P is provided by Ontario Health in support of Ontario’s Digital First for Health Strategy.

Amplify Care collects real-time information about which E2P tools you are using in your EMR and when, as well as feedback that you provide about our tools. This helps us understand value and impact and allows us to improve both the tool and the user experience. The data elements that we collect are limited to general information about tool usage and user, specifically:

Patient and clinician direct identifiers are hashed before they are collected by Amplify Care, and cannot be used to identify an individual. Other personal information (PI), such as your contact information, is only collected if you agree to provide it to us. Providing us with your usage data and feedback is voluntary.

Info we collect may be shared with external organizations, such as program co-leads, funding bodies (Ontario Health) and third-party evaluators, to support program evaluation, sustainability and future funding opportunities. The info will be aggregated before being shared and will not be associated with your identity. The collected info will be retained by Amplify Care for as long as required for the above purposes.

If you would like to stop providing us with your usage data at any time, please change your setting preferences in the “Usage Analytics” link within the tool, and going forward, your data will no longer be collected. This opt-out will not apply retrospectively.

Definitions

Hashing: Hashing involves applying a set of rules to protect the confidentiality of the message. Amplify Care uses a SHA-256 algorithm, which is a world-wide standard and is approved for use by both the Government of Ontario and the Government of Canada to protect sensitive electronic information.

Because hash values correspond to specific messages, they give Amplify Care a unique identifier for an individual, but one that is not personally identifying information. This allows Amplify Care to tell unique EMR users and EMR records apart, and to generate reliable reporting and metrics, all without ever viewing personal health information from the EMR.

Amplify Care commits to keeping the hash algorithm aligned with industry best practice.
