EMR searches for Pneumococcal Immunization

In partnership with the Association of Family Health Teams of Ontario (AFHTO), we have developed EMR searches for TELUS PS Suite, OSCAR Pro, and Accuro QHR, to identify adult patients who are eligible but have not yet had their pneumococcal immunizations.

These searches incorporate recommendations from the NACI 2023 guidelines, and will identify 3 groups of patients according to the following criteria:

Patients who are 65 years and older
Patients who are 50 to 64 with a specified medical condition or risk factor
Patients who are 18 years and older who are immunocompromised

There is a corresponding search and reminder for each of these groups of patients.

Pneumococcal Vaccine eligible – age 65 and over
Pneumococcal Vaccine eligible – age 50-64 with medical or risk factor
Pneumococcal Vaccine eligible – age 18 and over and immunocompromised
Pneumococcal Vaccine eligible – meets any criteria
Pneumococcal Vaccine eligible – age 50-64 with asthma
Pneumococcal Vaccine eligible – age 50-64 with COPD
