Practice Facilitation Program

The eHealth Centre of Excellence is excited to be partnering with the Health Innovation Group to deliver Canada's leading Practice Facilitation Program. Practice facilitation is an approach to supporting improvement in primary care practices that focuses on building organizational capacity for continuous improvement.

This program is suitable for anyone looking to support operational improvement within primary care practices, including: 

• Clinicians
• Nurse Practitioners (Program accredited by CNA) 
• Primary Care Office Managers 
• Change Management Specialists
• Digital Health Specialists

Program Delivery 

Virtual Model 

• Delivered 100% online

• Instructor-led, syncronous sessions

• Online sessions are instructor-led and synchronous - four (4) hours each 

• eight (8) online sessions spanning over three (3) months

• Cost: $5,495 CAD per participant (10% discount if registering three or more participants)

Hybrid 

• Combination of in-person and online delivery

• 2.5 days of in-person instruction (TBD)

• Online sessions are instructor-led and synchronous - 2.5 hours each 

• Number of online sessions: four (4)

• Cost: $5,995 CAD per participant (10% discount if registering three or more participants)
