We support Ontario Health Teams (OHTs) and Primary Care Networks (PCNs) across the province by offering Integrated Care Managers from our team who can assist your OHT with implementing your integrated care pathways and enabling those pathways with digital health tools. Advise OHT members regarding options and availability of digital health solutions and assist leadership on setting priorities

Lend digital health expertise to the priority patient population working groups (e.g., supporting clinical workflow mapping) as requested by the OHT
