We collect basic website visitor information on this website and store it in cookies. We also utilize Google Analytics to track page view information to assist us in improving our website.
The electronic referral (eReferral) proof of concept came to a conclusion this past summer, and the Ocean eReferral Network officially went live across the Waterloo Wellington region in August 2017. Since this time, over 600 eReferrals have been sent from primary care and other health care providers using the system! For more information on how you can access Ocean, please click here.

In the news: Canadian Healthcare Technology featured an article on the innovation procurement process that we used to select a vendor to design and deploy our eReferral solution, and an article also ran in the Kitchener Post and the Waterloo Record that highlighted both the central intake and eReferral model that we are currently supporting.
In October, our program was honoured to be recognized by the Association of Family Health Teams of Ontario (AFHTO), who gave us a Bright Lights award in the category of Optimizing Use of Resources!
